Peggy - Another possiblity is from Polishing the Die during the life of it's use and wearing the die down. This happens in Coin production at the US Mints also, its cheaper to polish a die as it becomes scratched etc. than to replace it. Polishing is also the reason we have Frosted Strikes and then non frosted stikes.
