I didn't get into this discussion sooner because we have been out and about on the road. When the gold spike got their machine they only had the one strike. It is believed by several other strike collectors that the "error" was created in the back room of that casino to increase the rate of play. It is not hard to punch out the center and place it back in and some collectors feel that their error shows this kind of damage. This has not been proved and is strictly an opinion of a few. I don't think the mint delivered any "errors" or that they had a bad quality control. That is just my opinion and we all have one of those as the saying goes. I have one of the "errors" in my collection and believe it is part of the silver strikers history and value it as such. I personally have not found any with a "loose" insert. Mine have all been pretty tight. Just my two cents.
